The form is confidentially received by the mentor liaison, who will contact you and discuss the next step. You will have the opportunity to be involved in finding a match that fits what you are looking for. Even after your form is turned in, there is no obligation to continue at any point.We look forward to helping you find someone to connect with! There is no obligation to turn in this form that requires you to meet with anyone.

- By joining this program, you are under the umbrella of West Hills Community Church, and therefore our mentors must abide by its statement of faith and beliefs. This ministry is not a replacement for professional counseling or teaching and must be treated as such. Our pastors, elders, and staff counselors are here to help confidentially with any complex situations that may require more experience or professional involvement. Although the relationship you form with your mentor will be confidential, our mentors reserve the ability to anonymously ask for advice or disclose information if it falls under the category of reportable information or if there is a pattern of unrepentant sin that requires the Matthew 18 approach for further church assistance.
